Our medical records team will contact you when your records are prepared. Once your records are ready:

• You can pick up a printed copy of your records at our local clinic.
• We can fax your records directly to another location if you have provided the fax information.
• Email your records from our secure portal – when you receive the message from our secure portal you will click on the link and follow the simple instructions to access your documents.

*Our fee for medical record request is $25.00. Completed forms will be released within 3-5 days business days.
